The College of St. Scholastica
Consent Form
Dear Participant,
We, Thomas Jezierski and Kasey Bacso, are undergraduate students in Dr. Karen Petersen's Psychology Empirical Research course at The College of St. Scholastica. We are offering you an invitation to participate in our research project, but you are under absolutely no obligation to participate.
The research study to which you have been asked to contribute involves: Reading a brief description of a sports situation associated with your specific sport, and fill out two questionnaires about how you feel after reading the description. Once you have completed this, you will read about another sports situation and answer the two questionnaires again. This will take approximately 20 minutes of your time.
Simply stated, not everything about the research project can be explained to you ahead of time. Yet we can assure you that there are no physical risks. The minimal psychological risks involved include possible discomfort when reading about situations related to your athletic activity and answering questions about yourself. We would like to remind you that if you are ever not comfortable in completing the study, you may withdraw at any time.
We will protect your personal identity by not including your name in any of our data. Only the accumulated results of the study will be accessible for the public to see.
You may discontinue at any time. We will not be able to tell you your individual results. If you are curious to see the overall results at the completion of the study, the abstract will be posted online under the "Student Research Projects" link on the College of St. Scholastica Psychology Department webpage, http://www.css.edu/Academics/School-of-Sciences/Psychology-and-Sociology.html. You may also revisit the consent form for this project at any time on the same link. If you have any questions or concerns about this research project please talk with Dr.Karen Petersen, (723-6031) or DRB chair Dr. Angela Hauger, (723-6148).
Thank you for taking the time to participate in our study,
- Thomas Jezierski and Kasey Bacso
