RN to BS Online

Upon acceptance into the RN to BS Online Nursing Track and prior to taking Community Health Clinical course, students will need to:

• Submit Release of Information Form
• Submit Immunization History Form
• Submit current 2-Step Mantoux
• Submit proof of coverage by professional liability insurance ($1 million for each
  occurence)
•  Submit a copy of CPR certifcation that includes expiration date
• Pass a state-mandated Criminal Background Check
