An individual 16 years of age or older who may have difficulties associated with a disgnosis such as:
- Traumatic Brain Injury
- Stroke
- Spinal Cord Injury
- Multiple Sclerosis
- Muscular Dystrophy
- Hand/Arm/Shoulder Injury
- Arthritis
- Age related Problems
- Neuromuscular Disorders
- Parkinson's Disease
- Cerebral Palsy
A person 16 years of age or older who may experience any of the following:
- Difficulty completing daily living skills including
- Cooking
- Cleaning
- Grooming/Hygiene
- Dressing
- Eating
- Driving
- Shopping
- Money Management
- Medication Management
- Home Maintenance
- Telephone Use
- Problems with shouder/arm or hand function
- Difficulty with mobility
- Decreased strength or endurance
- Needs assistance preparing for employment
- Functional Independence
- Joint Range of Motion
- Muscle Strength
- Cognitive Abilities
- Independent Living Skills
- Gross and Fine Motor Skills
- Visual Deficits
