    This survey is used as a guide for students as they document background information that will be shared with cooperating teachers.

    The field experience student will use this framework to design lessons for all formal observations by the cooperating teacher and college supervisor.

    This form is used during secondary student teaching. It assesses the Standards of Effective Practice, which are embedded in the course.

  • Professional Behaviors Form

    This is an evaluation of a students Professional Behaviors. It is to be completed by the cooperating teacher and forwarded directly to the college.

  • Context for Learning Form

    Used as required in your field experiences. Students will include information on demographics, community context, physical environment, instruction and learning environment.
